Auto-Continuation Settings Form

Effective Date:
This agreement takes effect upon signature. 

Automatic Continuation Orders:
Opt-In: By signing this agreement, you opt into automatic continuation orders.

Process:
The patient's case must be tracked in Dental Monitoring. When each case reaches the 2nd or 3rd to last aligner step in a batch, Orthosnap will evaluate the Dental Monitoring scans. If the scans indicate no changes in the treatment plan are required, your clinical account manager is authorized to order the continuation batch of the same size. If the patient is not tracking well, the automatic continuation order will not be placed. You will be notified that remediation actions are necessary, such as a new scan or clinical consultation.

Notification and Response:
Orthosnap will email the doctor and practice email address on file to inform them of the continuation order. The practice has a 24-hour grace period to request modifications, pause, or cancel the order. If no response is received, the continuation order will be placed, and the treatment plan for the next batch will be created and aligners produced.

Billing and Financial Responsibility:
The practice will be invoiced once the continuation order is placed. The practice is financially responsible for the cost of the continuation order once the order is placed.

Dispute Resolution:
In the event of a dispute, the practice owner agrees to pay for the cost of a new batch of aligners and waives all claims against Orthosnap regarding the reordering of this new batch, per this agreement.

General Terms:

Duration and Termination:
This agreement is valid until either party opts to cancel. Contact Information: The account email address will serve as the primary point of contact for all communications and modifications.

Governing Law:
This agreement is governed by the laws of New York State. Amendments: Orthosnap reserves the right to change this agreement as it sees fit. Liability Limitations: Orthosnap is not responsible for late replies from the practice.
